Effective the week of January 19, 2026, we will no longer collect patient names or dates of birth. This change will enhance patient privacy and streamline your intake experience.
Action Required for Repeat Patient Consultations: Because we will no longer use name or date of birth identifiers to track patients, we are moving to a Record ID system to help maintain continuity of care.
- Look for the ID: Every PCL consultation note we send you will include a unique PCL Record ID number.
- Have it Handy: If you call back regarding the same patient, please try to have the PCL Record ID # or the previous consultation note available. This will help us more quickly locate and reference previous recommendations.
